Raphael M. Barishansky, a consultant, is a former Pennsylvania deputy secretary of health. Previously, he served as director of the Office of Emergency Medical Services for the Connecticut Department of Public Health; chief of public health emergency preparedness and response for the Prince George’s County, Md., Health Department; and executive director of the Hudson Valley Regional EMS Council.

Barishansky holds a doctoral degree in public health as well as master's degrees in public health and homeland security studies. His publications have appeared in various trade and academic journals, and he is a frequent presenter at state, national and international conferences.

The causes of these alarming gaps in equitable access to emergency care are complex. Fixing the problem won’t come from patchwork efforts or temporary fixes.
Designating them by law would go a long way toward addressing the many issues these critical services face. They have evolved over decades to encompass a multitude of responsibilities.
The meaning has evolved, but actions need to evolve as well.
